Sunderland GP Alliance were successful in becoming one of 73 NHS England pilot sites in 2016, encompassing thirteen GP practices split across the city’s five localities. The SGPA employs one Senior Clinical Pharmacist and six Clinical Pharmacists, funded though NHS England and the CCG with an increasing practice contribution over 4 years to provide a collaborative at scale Pharmacist resource.
Pharmacists are embedded within the Practice teams resolving day to day medicine issues and delivering direct patient care including managing long term conditions and providing advice to patients on multiple medications and improving access to health professionals.
The SGPA are taking learning from the pilot to develop an extended at scale model across the whole city, improving career prospects for Pharmacists and providing Practices with a sustainable and affordable Pharmacist resource.
