Sunderland Healthcare Pathway is a system that improves patient access by signposting best practice / locally agreed clinician responsibilities to ensure the patient is treated by the right clinician at the right stage of their illness / condition.
It ensures the central, co-ordinating role of general practice in delivering out of hospital care by enabling primary care clinicians to easily see locally agreed / national guidance at the point of referral. This is turn encouraging new working arrangements by signposting how to access enhanced primary care services alongside existing services. Health pathways also Supports Medicines Optimisation objectives by integrating locally agreed treatment guidelines into web pages, and Supports the CCG Standardisation of Care Objectives where there are known issues regarding variation of referral criteria etc.
