About the service


The GP Alliance employs a team of Clinical Pharmacists and Pharmacy Technicians who work across Primary Care Networks (PCNs) to deliver high-quality pharmacy services through a city-wide model.

Our team supports a range of medicines-focused workstreams tailored to the needs of local GP practices.

A key part of our offer is the Medicines Discharge Hub, led by Pharmacy Technicians, which ensures safe and efficient medicines management following hospital discharge.

We’ve developed a fully integrated and collaborative approach to clinical pharmacy and medicines optimisation within primary care.

Our pharmacy team plays a vital role in resolving day-to-day medicines issues, supporting patients directly, and contributing to safer, more effective prescribing. By working closely with practices and patients, they help manage complex medication regimes and improve outcomes for people with long-term conditions.
