The new Service Level Agreement for GP input into Community Integrated Teams is into its final month of the first quarter with almost all Practices now signed up and working to the new arrangements.
MDT Coordinators continue to meet with Practices to offer support and clarity in delivering the new agreement and we continue to see a rise in the number of care plans in place month on month. Practices who have not yet met with their Coordinator are invited to do so and are encouraged to sign up to the SLA as soon as possible to ensure that full funding allocations are achieved.
