Map of Medicine

The installation programme for Map of Medicine continues across the city, as we said last month if you haven’t already got Map you will be contacted by a company called USS to book your installation.

This shouldn’t take long and should be done with minimal disruption over late mornings and dinner times. We will be contacting all the surgeries to ensure training is booked to enable you to use the system. The training will take place on 21st July and 28th July and we have a Go Live date which will be after the September TITO so please watch this space.

Care Home Re-Alignment

As described last month the care home re-alignment work is now getting underway in the West locality, I have been to the East locality to discuss this with them as they make a natural next step when you look at the distribution of care home patients across those two localities.

We have a plan to move at pace across the city to minimise any periods of time where new patients are being registered before other patients are moving to their new aligned surgery.

SGPA Community Ambulatory ECG Service

We have now launched our ambulatory ECG service within the 5 localities across Sunderland.

As a reminder the hubs are:

Referrals are made through the ECG Cloud link which you can put onto your desk top and you print out the referral and give to the patient. You can make the appointment for the patient or get them to do this, there will be a list of local hub sites on the referral. The patient’s report will then be returned to the central practice email address. There is an e-consultation process if you require further guidance from a consultant cardiologist, this is accessible via the ECG Cloud with a user name and password which your Practice Manager can create or Technomed will help if you call their customer support number.

SGPA Insulin Initiation Hub Service

Also at TITO we launched the insulin initiation hubs.

There is one of these per locality:

Over the coming weeks these practices will be able to take a referral using the template shared via email of any patient who meets the inclusion criteria. Please look out for the locality specific email which has more details of when each hub will be ready to take your referrals and how to access the service

I know these services are new and require you to change your normal practice. They are designed to give us an opportunity to experience working collectively and collaboratively as GPs so I’d like to encourage you to use them.
