Just to let you know the Post Discharge Pilot is now under way:
Clinical Projects Manager Karen Swaile has been based in The Galleries HC during April using the post-discharge information received in Sunderland GP Alliances own practices from CHS and QE Hospital Gateshead to test the process. It is very early days but we will be receiving all the practice data from those involved in the pilot during May, with a more informed update in our June newsletter.
