This pilot will start from the 24.04.2017 in the Washington Locality.
Karen Swaile will be in touch with those practices that are directly involved during the next 4 weeks, the process will be re-visited to ensure that no amendments are required to the original format. As a reminder this pilot was to try to establish if we could more proactively manage those patients who have had a complex acute medical discharge by reviewing and signposting their care more appropriately following hospital discharge to prevent re-admission and release GP capacity. A small clinical team including a pharmacist, led by Karen (an experienced registered nurse) will undertake this pilot and report back the outcomes and experience.
